Support Team Network

Faith in Action

 

Guilford Park is a member of the community partnership for End of Life care, a ministry which provides End of Life Care Ministries in the local congregations.  "Faith in Action" is a national program of the Robert Wood Johnson Foundation administered by the Wake Forest University School of Medicine.

Through it, our members are trained to work with families facing end of life issues including advanced directives, caregiving, and bereavement support.  Our focus is to honor patient/families wishes about the kind of care they desire at the end of life.

We offer periodic workshops and training sessions in filling out advanced directives, memorial service planning, caregiving issues, long term care, medicare and Medicaid planning, and other related issues.

Congregational Health

This program is part of the Moses Cone Health System Congregational Nurse, volunteer model.  A health committee provides direction.  Health topics or issues are placed on a designated "Congregational Health" bulletin board each month.  Health related programs and tips are printed in our newletter monthly. Screening for blood pressure and blood glucose are conducted as needed and upon request from members or visitors of the church.  Nurses are available when needed to assist the congregation in any health related issues, upon request.  For more information, contact Elaine Button,  elainebutton@triad.rr.com
